We are seeking an experienced Clinical Nurse Consultant to provide expert clinical leadership and care coordination within our Complex Care Program. This fixed-term opportunity offers the chance to step into a senior clinical role supporting clients with complex and/or chronic disease.
The Contract: Fixed Term, Part Time, 56 hours per fortnight (21st September 2026 - 4th April 2027). Shift times are 8.00 am - 4.30 pm, Monday to Friday.
About the Program
The Complex Care Program provides specialised, client-centred care coordination to people with chronic and complex conditions, aiming to improve outcomes and reduce demand on acute services.
The Position:
In this role you will facilitate service access, undertake comprehensive client assessments and coordinate the development and implementation of multi-disciplinary, goal-based care plans. You will ensure best practice in care coordination and service delivery to support client-centred outcomes, promote health independence and reduce avoidable hospital presentations.

What You Bring:
- Current APHRA registration as a Registered Nurse
- Minimum 5 years acute nursing experience
- Demonstrated experience in chronic or complex disease management with advanced clinical assessment skills
- High-level skills in client and carer education within a chronic disease framework
- Strong understanding of community services that support health independence
