Take the next step in your nursing career with this senior clinical opportunity supporting people living with chronic and complex health conditions. As a Clinical Nurse Consultant, you'll use advanced assessment, care coordination and clinical leadership skills to improve health outcomes and support client independence. This is an excellent opportunity to further develop your expertise in chronic disease management within a specialised community-based role.
Key Responsibilities
- Conduct comprehensive client assessments
- Coordinate multidisciplinary care plans
- Support chronic disease self-management
- Provide client and carer education
- Facilitate access to health and community services
- Collaborate with hospital and community providers
The Contract: Fixed term opportunities available
56 hours per fortnight to 4th April 2027 | 32 hours per fortnight until 3rd January 2027
Skills & Experience
- Current AHPRA registration as a Registered Nurse
- Experience in chronic or complex disease management
- Advanced clinical assessment skills
- Strong care coordination and education capability
- Ability to work effectively in multidisciplinary team
