Latrobe Regional Health (LRH) is looking for Community Mental Health Navigator to join our growing team on a full-time fixed term basis until October 2026 for parental leave coverage.

 

The Community Mental Health Navigator role is designed to complement the work of clinicians and peer workers by providing practical, non-clinical support to consumers and their families. As a Mental Health Navigator, you will help people at risk of relapse, disengagement or recent crisis to stay connected with treatment, navigate health and social systems, and access the services they need.

The role of the Community Mental Health Navigator is:

  • Welcoming and orientating consumers and families to community mental health services.
  • Assist with appointment booking, reminders, and follow up phone calls.
  • Supporting consumers and careers to access health, social, and community services.
  • Providing general information about welfare, housing, employment, and NDIS supports.
  • Maintaining accurate documentation and escalating concerns to clinical team.

 

About you

Ideal candidates for this role will have:

  • A recent graduate health practitioner in nursing, social worker or occupational therapy.
  • Compassionate, adaptable, and committed to supporting people experiencing mental illness.
  • Skilled in communication and interpersonal engagement with strong customer service focus.
  • Organised, with the ability to manage schedules, logistics, and administration.
  • Comfortable working both independently and collaboratively in a team environment.
  • Proficient in Microsoft Office and general IT systems.
