Latrobe Regional Health (LRH) is looking for a Community Mental Health Navigator to join our growing team on a fixed-term part-time basis (0.7EFT). This is an exciting opportunity for a recently graduated health professional to build a career in mental health while supporting consumers to stay engaged with services and achieve positive outcomes.
About the Role
The Community Mental Health Navigator plays a vital role in supporting consumers, families and carers to navigate the health and social care system. Working as part of a multidisciplinary team, you will provide practical, non-clinical support that helps individuals remain engaged in their care and access the services they need.
You will be involved in welcoming and orienting consumers to the service, coordinating appointments, providing follow-up support, and assisting with access to community resources such as housing, employment, and the NDIS. This is a dynamic, community-based role with a strong focus on engagement, communication, and recovery-oriented practice.
Key Responsibilities
- Support consumers and carers to navigate mental health and broader health services
- Coordinate and confirm appointments, including follow-ups for non-attendance
- Provide phone-based and face-to-face engagement to promote service participation
- Assist with referrals to community supports including NDIS, housing and welfare services
- Liaise with internal teams and external agencies to support continuity of care
- Maintain accurate and timely documentation
- Identify and escalate risks or changes in consumer wellbeing to the clinical team
- Contribute to a collaborative multidisciplinary team environment

About you
You are a motivated and compassionate early-career professional with a strong interest in mental health and community care. You thrive in a team environment, communicate effectively, and are highly organised with the ability to manage competing priorities.
You will also have:
- A recent qualification in Nursing, Social Work or Occupational Therapy
- Excellent interpersonal and communication skills, including telephone engagement
- Strong organisational and administrative skills
- Ability to work collaboratively and follow direction within a team
- A genuine commitment to recovery-oriented, consumer-centred care
- A current Victorian driver’s licence
