Latrobe Regional Health is looking for HIP Care Coordinator to join our growing on a part time (0.5EFT) ongoing basis.

 

The Health Independence Program (HIP) Care Coordinator plays a vital role in supporting patients to achieve improved health outcomes following discharge from hospital. Working within the Health Independence Programs (HIP) team at Latrobe Regional Health, you will assess individual needs, coordinate services and manage a caseload of clients requiring ongoing support through community-based programs.

 

Primarily working within the Hospital Admission Risk Program (HARP), with exposure to the Transition Care Program (TCP) and Residential In-Reach (RIR), this role focuses on preventing avoidable hospital presentations, supporting chronic disease management and coordinating care across acute, community and aged care services.

The role of the HIP Care Coordinator is to:

  • Assess, coordinate and manage a caseload of clients enrolled in Health Independence Programs to support improved health outcomes and independence.
  • Develop, implement and evaluate individualised care plans for patients with chronic conditions, complex health needs and risks of hospital admission.
  • Coordinate care across acute, community, specialist and aged care services to ensure seamless transitions and continuity of care.
  • Provide clinical assessment, education and support to clients and their families to promote self-management and wellbeing.
  • Work collaboratively within an interdisciplinary team to deliver evidence-based care aligned with chronic disease management principles.
  • Contribute to quality improvement initiatives, service development and the delivery of safe, person-centred care.

 

About you

Ideal candidates for this role will have:

  • Current registration as a Division 1 Registered Nurse with AHPRA and a current Victorian Driver Licence.
  • Minimum five years post-graduate experience in an acute healthcare setting, supported by recent clinical experience and ongoing professional development.
  • Demonstrated experience in patient assessment, clinical decision-making and the management of clients with complex health needs.
  • Strong communication, interpersonal and relationship-building skills, with the ability to work effectively within interdisciplinary teams.
  • Knowledge of evidence-based chronic disease management principles and a commitment to delivering person-centred care.
  • Well-developed critical thinking, problem-solving and organisational skills, with the ability to manage competing priorities.
