Latrobe Regional Health is looking for Health Care Worker to join our growing team on a part time (0.8EFT) fixed term basis for 12 months and casual opportunity available.

 

We are seeking a Health Care Worker to provide essential personal care and domestic assistance to patients across Post-Acute Care (PAC), the Transitional Care Program, Transition to Home, and Hospital in the Home (HITH). This role delivers direct support in patients’ homes, helping individuals maintain independence, safety and comfort while they recover in the community.

The role of the Health Care Worker is to:

  • Conduct home visit safety assessments to ensure a safe care environment.
  • Provide personal care support including showering, bathing (including bed baths), dressing, grooming, mobility assistance, repositioning and prompting medications (not administering).
  • Deliver domestic assistance such as cleaning, vacuuming, mopping, bathroom and toilet cleaning, making beds, washing linen, kitchen cleaning, and supporting shopping tasks.
  • Complete accurate and timely documentation related to care delivery and patient needs.
  • Restock non-emergency equipment and maintain care equipment in a clean, functional condition.
  • Support patients respectfully, compassionately and in alignment with LRH policies, safety requirements and organisational values.

 

About you

Ideal candidates for this role will have:

  • Certificate III in Individual Support or Certificate IV in Ageing Support or equivalent support.
  • Current experience providing care in community, aged care or hospital settings.
  • Current drivers’ licence and ability to work in patient homes across the region.
  • Ability to work both autonomously and as part of a supportive team.
  • Demonstrates integrity, communication skills, teamwork, adaptability, compassion, empathy and respect.
