Latrobe Regional Health is looking for Oncology Clinical Coordinator – Frailty Screening Project to join our growing team on a part time (0.4EFT) fixed term basis for 12 months.

The Oncology Clinical Coordinator – Frailty Screening Project leads the implementation and evaluation of a nurse‑led frailty screening and geriatric assessment program within the Oncology Service. The role focuses on improving outcomes for older adults with cancer through the integration of evidence‑based frailty screening tools into routine clinical practice.

Working collaboratively with multidisciplinary teams, the position coordinates clinical implementation, education, referral pathways, data collection and quality improvement activities to establish a sustainable, frailty‑informed model of cancer care.

The role of the Oncology Clinical Coordinator – Frailty Screening Project is to:

• Implement validated frailty screening tools and coordinate comprehensive assessments for older oncology patients identified as frail or vulnerable.
• Coordinate referrals to geriatricians and supportive care services, ensuring timely access to appropriate interventions and multidisciplinary care.
• Lead the rollout of the frailty screening program, including development of clinical workflows, documentation processes and integration into existing oncology pathways.
• Deliver education, training and clinical support to nursing and multidisciplinary staff on frailty screening, geriatric assessment principles and frailty‑informed oncology care.
• Monitor program performance through data collection, analysis and reporting on key measures including screening rates, referrals and patient outcomes.
• Contribute to quality improvement, audit and project evaluation activities, supporting continuous enhancement of oncology services and patient care.

About you

Ideal candidates for this role will have:

• Current registration as a Registered Nurse with AHPRA.
• Experience in clinical assessment, care planning and coordinating patient‑centred care.
• Demonstrated experience in quality improvement, clinical projects or service implementation initiatives.
• Strong organisational and time‑management skills, with the ability to work autonomously and manage competing priorities.
• Computer literacy and experience using electronic medical records and reporting systems.
• Oncology and/or haematology nursing experience, postgraduate qualifications in Cancer Nursing or Gerontology, and experience in frailty screening, education, research or project evaluation (desirable).
