Latrobe Regional Health is looking for Residential in Reach Care Coordinator to join our growing team on a part time (0.5EFT) fixed term basis for 3 years.
We are seeking a motivated Nurse Practitioner Candidate – Aged Care to join our Residential in Reach (RIR) team. This advanced practice role supports residents in aged care facilities by providing timely assessment and management in their home environment, reducing unnecessary hospital transfers and improving continuity of care. As a candidate, you will undertake a structured mentorship and supervision program while completing your Nurse Practitioner studies, developing leadership, clinical expertise, and advocacy skills required for endorsement by Nursing and Midwifery Board of Australia.

The role of the Residential in Reach Care Coordinator is:
- Respond to referrals and enquiries in line with Non-Admitted Patient Activity guidelines and timeframes.
- Triage and review residents in aged care facilities, performing advanced clinical assessments.
- Develop and implement management plans in consultation with General Practitioners and other health professionals.
- Complete accurate and timely documentation and participate in family meetings as required.
- Collaborate with multidisciplinary teams and external organisations to ensure integrated, patient-centred care.
- Contribute to evidence-based practice, education, and quality improvement initiatives within the service.
About you
Ideal candidates for this role will have:
- Registered Nurse with current AHPRA and enrolled in a Master of Nursing (Nurse Practitioner).
- Minimum 5 years post graduate nursing experience in an acute setting.
- Exceptional clinical assessment and decision-making skills with advanced knowledge in managing complex care needs.
- Strong communication, stakeholder engagement, and teamwork skills.
- Current Victorian Driver’s licence.
- Commitment to best practice and patient safety.
- Experience in research, teaching, and quality improvement activities (desirable).
