We’re currently seeking a Grade 2 Social Worker who thrives in a dynamic, hospital-based environment and is passionate about contributing to positive patient outcomes and safe transitions of care.

 

About the Role

As a Grade 2 Social Worker, you’ll provide high-quality psychosocial care to patients, families and carers across acute and subacute hospital settings. This rotational role involves complex case management, crisis intervention, advocacy, discharge planning and multidisciplinary teamwork.

 

You’ll also take an active role in mentoring junior staff and students, contributing to a positive, psychologically safe team culture, and participating in service improvement initiatives.

 

Key Responsibilities

  • Deliver trauma-responsive, culturally safe social work aligned with AASW Code of Ethics and relevant legislation.
  • Provide psychosocial assessments, brief interventions and complex discharge planning for patients with diverse needs.
  • Mentor and support Grade 1 Social Workers, students and Allied Health Assistants.
  • Collaborate across multidisciplinary teams to improve patient experience and outcomes.
  • Contribute to quality improvement, guideline development and professional supervision.

 

About you

Ideal candidates for this role will have:

  • Bachelor or Qualifying Masters in Social Work and eligibility for full AASW membership.
  • Demonstrated experience providing social work services in hospital or health care settings.
  • Sound clinical skills in crisis intervention, complex discharge planning and psychosocial assessment with diverse populations, underpinned by the AASW Code of Ethics.
  • Ability to prioritise a dynamic caseload and work effectively under pressure.
  • Experience mentoring or supervising students or junior colleagues.
  • Strong interpersonal and communication skills, including capacity to build respectful relationships and work collaboratively in multidisciplinary settings.
