Latrobe Regional Health (LRH) is looking for a Transition to Mental Health Registered Nurse to join our growing team on a full-time ongoing basis.

 

About the Role

 

The Mental Health Transition Program offers Registered Nurses an excellent opportunity to develop and consolidate their skills as competent and confident mental health practitioners through a 12-month program. The program provides structured education and clinical experience to support the transition into specialist mental health nursing practice

 

The Mental Health Transition Clinician will develop the skills, knowledge and attributes required to provide high quality, safe, trauma-informed, recovery-oriented, and evidence-based mental health nursing care. The role contributes to achieving positive consumer outcomes across Latrobe Regional Health’s Mental Health and Wellbeing Services and partner agencies.

 

Key Responsibilities

 

  • Undertake timely, comprehensive assessments including risk assessment and formulation of individualised care plans.
  • Participate in regular clinical reviews in collaboration with consumers, carers, significant others and the multidisciplinary team.
  • Ensure care planning documents are completed, evaluated and updated in collaboration with consumers, carers, significant others and the multidisciplinary team.
  • Monitor and manage adverse effects of medication in line with clinical guidelines.
  • Provide pharmacological and psychoeducation tailored to consumer needs.
  • Respond effectively to psychiatric or medical emergencies in accordance with policy and procedure.
  • Facilitate and co-facilitate group therapy sessions and recovery-focused programs, including daily living skills and activity groups.
  • Support orientation of new staff and students, providing supervision and mentorship as appropriate.
  • Contribute to safe and coordinated discharge planning in collaboration with the multidisciplinary team.
  • Maintain accurate and timely clinical documentation.
  • Attend study days and complete learning activities as outlined in the training matrix.

About you

 

To be successful, you will have:

  • Registration with AHPRA as a Division 1 Nurse with experience in Acute Nursing
  • Sound clinical knowledge and skills
  • Strong communication and interpersonal abilities with a recovery-oriented approach
  • Ability to work collaboratively in a multidisciplinary environment
  • Current Victorian Driver’s Licence

 

Desirable:

  • Minimum 2 years in an acute nursing role
  • Awareness of the Royal Commission into Mental Health and the expected changes to practice and service delivery
  • Commitment to reflective practice
