At Latrobe Regional Health (LRH), we are committed to delivering accessible, high-quality mental health services across our community. We are seeking a Community Mental Health Navigator to join our dedicated Mental Health team. This role plays an important part in supporting consumers and their families by helping them navigate complex health and social systems, ensuring they feel welcomed, informed, and connected to the right services.
This position offers an excellent opportunity for recently graduated health practitioners (nursing, social work, or occupational therapy) who are passionate about mental health and are eager to begin a rewarding career in the sector.
Key Responsibilities
In this role, you will:
- Support consumers and their families to navigate mental health and social service systems, including welcoming and orientating people to the clinic.
- Assist with appointment scheduling, reminders, and follow-ups to help reduce barriers to engagement.
- Provide information and linkage to external services such as housing, NDIS, employment, and community organisations.
- Actively participate as part of a multidisciplinary community mental health team.
- Maintain accurate documentation and escalate concerns about consumer wellbeing to the clinical team where required.
About You
You are a compassionate and motivated professional with:
- A recent qualification in nursing, social work, or occupational therapy.
- Strong interpersonal and communication skills, including excellent phone manner.
- Well-developed organisational and time management skills.
- The ability to work both independently and as part of a team.
- A current Victorian driver’s licence.
