South Gippsland Hospital (SGH) is a public rural health service providing integrated acute hospital and community care services. Care is delivered through our Community Health Centre, Banksia Centre, and in clients’ homes across the region. In partnership with the doctors of Foster Medical Centre, SGH supports the health and wellbeing of the Foster community and surrounding areas, including Wilsons Promontory. South Gippsland Hospital (SGH) is seeking a motivated Social Support Group Worker to join our team.
The SGH Social Support Group is designed to enhance people’s independence by promoting physical activity, cognitive stimulation, good nutrition, emotional wellbeing and social inclusion for older people.
The position contributes positively as a member of the Social Support Group team to assist in the provision of high quality activity programs, including group based and individual social activities for clients who attend the service.
To be successful in this role, you will have:
- Appropriate qualifications with a commitment to further education as required.
- Demonstrated experience working with groups and conducting group-based activities (desirable).
- Demonstrated experience working with older people in a community setting and a commitment to enhancing the quality of life of older people.
- Demonstrated ability to empower clients to participate in decision making and activity planning.
- Proven ability to monitor and observe clients’ well-being and behaviour and escalate concerns as required.
- Well-developed interpersonal, communication and organisational skills, including ability to use Microsoft Office software and other computer software as required.
- Demonstrated ability to work within a team.
- Proven ability to maintain high quality documentation including client notes, program documentation, care planning and incident reporting.
Essential:
- Relevant Tertiary qualification (with a minimum of two years study, or equivalent combination of qualifications and relevant experience recognised by the employer): Certificate IV in either Aged Care, Community, Leisure and Lifestyle, Individual Support, Diversional Therapy, Nursing or Allied Health Assistant qualifications
- Current non-restricted Victorian Driver’s Licence.
- Ability to use Microsoft Office software and other electronic communication and client records management software.
- Minimum 2 years’ relevant experience post certificate.
Desirable:
- Current First Aid Certificate or willingness to work towards same.
- Current Food Handlers Certificate or willingness to work towards same.
