Support at Home Service Coordinator
Grade 2
Casual – Immediate Commencement
The primary role of the Support at Home (SAH) Service Coordinator is to assist Support at Home Care Partners to provide continuity of services that assists clients to remain living independently in their own homes. The role requires:
- Coordination and organisation of services, allocated by care partners in consultation with clients, including new referrals, internal services and third-party providers.
- Well-developed understanding of services available to effectively assist clients to remain living independently in their own homes
- Completion of intake, admission and ongoing data entry for SAH clients including new referrals.
- Close collaboration with SAH Care Partners and the SAH rostering team.
To be suitable for this role you must have:
- Demonstrated empathetic, respectful, and professional attitude, adhering to high ethical standards.
- High level IT, decision-making and organisation skills, including the ability to work autonomously, manage competing priorities and meet deadlines.
- Well-developed communication skills.
- Demonstrated knowledge of the Aged Care Quality Standards.
- Strong understanding of services relevant to this client cohort.
- Experience working with aged community-based clients and knowledge of the range of issues experienced by this client cohort.
