Transition Care Program (TCP) Care Partner Casual

Welfare Worker, Class 2A

We invite existing, suitable qualified employees to apply the role of Transition Care Program (TCP) Care Partner – casual. The primary role of the Transition Care Program (TCP) Care Partner is to ensure continuity of care co-ordination to assist frail older people, and younger people with early entry approval to the Aged Care system, and their carers, to remain at home and in their communities, in accordance with current Commonwealth TCP Guidelines.

The role requires:

  • well-developed needs and risk assessment, consumer directed care planning and case management skills,
  • workforce management skills,
  • thorough understanding of contractual and statutory requirements relevant to the role
  • experience in aged and community care, assessment and goal directed care planning based on a consumer centered model of health.

 

The successful applicant will demonstrate the following attributes:

 

  • Relevant tertiary qualifications and experience
  • Demonstrated empathetic, respectful and professional attitude, adhering to high ethical standards.
  • High level IT, decision-making and communication skills, including the ability to work autonomously and meet deadlines.
  • Demonstrated case management, care coordination and goal directed care planning skills.
  • Demonstrated understanding of rehabilitation and the principles and guidelines of the TCP program.
  • Effective networking and mutli-disciplinary team liaison capabilities
  • Experience working with aged consumers
  • Thorough understanding of the range of issues experienced by and services relevant to this client cohort.
