An exciting opportunity has arisen to join the District Nursing Team at West Gippsland Healthcare Group in the role of a District Nurse.

The District Nurse is an autonomous, skilled practitioner who delivers high-quality, evidence-based care to clients in their homes, supporting them to remain safe and maintain independence. This rewarding role offers a diverse and dynamic caseload, with opportunities to provide care across District Nursing, Hospital in the Home (HITH), and Palliative Care, enabling you to expand your expertise in acute, chronic and end-of-life care within the community.

Working with a high degree of clinical autonomy, the nurse undertakes comprehensive assessments, implements and evaluates care, and provides advanced interventions across a broad scope of practice. The variety within the role ensures ongoing professional growth, with exposure to acute, subacute and end-of-life care in the community.

The position involves close collaboration with multidisciplinary teams and strong engagement with clients and carers to promote self-management, wellbeing, and coordinated, person-centred care. This is an excellent opportunity for a clinician seeking a fulfilling role that offers both variety and the chance to make a meaningful difference in the lives of patients and their families.

 

Our preferred candidate will have:

  • Current Registration with AHPRA.
  • Minimum 3 years post-registration acute clinical experience
  • Demonstrated ability to work independently with minimal supervision
  • Strong clinical skills in assessment, care planning, and evaluation
  • Excellent time management, prioritisation, and organisational skills
  • Effective communication and interpersonal skills with patients and multidisciplinary teams
  • Current driver’s licence

 

The following experience/qualifications would be considered advantageous:

  • Previous experience in Community or District Nursing.
  • Competence in complex clinical interventions including CVAD’s, IV Cannulation, wound management and catheter management
  • Experience in chronic disease management and palliative care
  • Postgraduate qualifications in community, chronic disease, wound management or other District Nursing related fields
  • Experience in client education and promoting self-management
  • Leadership, mentoring, or preceptorship experience

 

IMPORTANT: Some roles in the healthcare sector are required to be vaccinated annually for Influenza (unless a valid exemption applies). Evidence of vaccination (or exemption) will be required to be provided prior to commencement.
