About the role 

The Care Partner is responsible for individually planning and coordinating community aged care services in partnership with the client. This involves working closely with each client and their families to ensure they receive an individualised, responsive service under a consumer directed care model. The Care Partner will undertake assessments, develop care plans and manage an allocated client case load. The Care Partner will also be responsible for all client documentation and the maintenance of individual client budgets.

 

The Care Partner is responsible for managing a set case load of HCP clients as advised by the Director Primary Health Care. This role will promote YDHS services, and develop strategies to engage with prospective clients. The Care Partner will focus on internal service provision at YDHS as the priority to support sustainable growth of the health service.

 

Responsibilities

  • Meet with potential CHSP consumers and their families to provide information about available Commonwealth Home Support Programme services, eligibility criteria, service scope, and referral pathways.
  • Support consumers to understand and access appropriate aged care services through the My Aged Care system, including CHSP services and referrals to other supports where required.
  • Build strong working relationships with key YDHS services, particularly discharge planners, service access teams, and community service providers to increase awareness of CHSP services and streamline referral processes.
  • Ensure all client information is accurately entered into the YDHS database and client records in a timely and compliant manner.
  • Complete all required client documentation in accordance with organisational policies, procedures, and program requirements.
  • Ensure consumers are informed of their rights and responsibilities and provide accurate, up-to-date information regarding privacy, confidentiality, and the use or disclosure of personal information.
  • Inform consumers of applicable CHSP contributions or fees associated with accessing services, in line with organisational policy and program guidelines.
  • Provide care coordination and support to CHSP consumers to ensure services are delivered in accordance with identified needs and goals.
  • Monitor service delivery to ensure services remain appropriate and responsive to the consumer’s changing needs.
  • Apply appropriate support and intervention strategies including referral to Allied Health, Home Support services, Community Nursing, and external community services as required to support goals identified in the support plan.
  • Assist consumers to connect with broader community supports and programs to maintain independence and wellbeing.
  • Develop and maintain a working knowledge of local community services, agencies, and support networks to facilitate access and coordination of care.
  • Manage workload with a high level of autonomy while remaining responsive to program priorities and organisational requirements.
  • Participate in case discussions, care planning meetings, and multidisciplinary forums where required to ensure coordinated service delivery.
  • Maintain professional knowledge by keeping informed of current practices in community and aged care, attending mandatory training, and participating in professional development opportunities.
  • Uphold the consumer’s rights to privacy, confidentiality, autonomy, dignity, and safety at all times.
  • Work within the scope of the role and escalate concerns where appropriate.
  • Demonstrate organisational values of integrity, collaboration, acceptance, respect, and enthusiasm in all interactions.
  • Ensure provision of a high-quality service by adhering to all relevant legislation, policies, protocols, and standards.
  • Contribute to a safe workplace for colleagues and consumers.
  • Participate in performance review processes and undertake mandatory and role-related training.
  • Work with consumers to develop an individual Care Plan.
  • Perform other duties as directed by the Line Manager and Director of Primary Health Care.
  • Complete CHSP Service Agreements.

  • Ensure all consumers, their families, visitors, and colleagues are treated with respect and dignity.
  • Listen to and collaborate with consumers to understand their experiences and preferences, supporting flexible service delivery that promotes independence and wellbeing.
  • Support consumers to understand their health and available services.
  • Encourage consumer participation in decisions about their care and support.
  • Provide clear information about services using language that is easy to understand and free from jargon.
  • Respect the healthcare and lifestyle decisions made by consumers.
  • Ensure service delivery is free from discrimination based on age, gender, gender identity, sexual orientation, disability, employment status, cultural background, or religious beliefs.
  • Complete falls management/deteriorating client care process as per policy.

 

Key Selection Criteria

Mandatory:

  • Previous experience working within the health, community services, or aged care sector.
  • Experience supporting client intake, service coordination, or consumer assessments.
  • Sound understanding of person-centred care and community-based service delivery.
  • Understanding of health and social issues impacting older people and the ability to connect consumers with appropriate community supports.
  • Demonstrated ability to prioritise workload and manage competing demands while meeting program timeframes.
  • Demonstrated computer literacy including experience working within a Microsoft Windows environment, client management systems, and electronic information exchange.
  • Strong communication skills with the ability to build rapport with a diverse range of consumers, families, and service providers.
  • Minimum Australian Qualifications Framework (AQF) Level 4 qualification, such as a Certificate IV in Aged Care, Community Services, Disability, or a related discipline.

Desirable:

  • A degree or diploma in nursing or a health-related discipline preferable.
  • A thorough understanding of My Aged Care system and how it relates to Commonwealth Home Support Program
