Position Description
To provide home support services that are appropriate to the client’s level of physical functioning and their cultural norms; ensuring services are provided in a way that enhances and maintains an individual’s abilities and independence.
Responsibilities
- Provide home support services in accordance with each client’s Care Plan, ensuring care aligns with the Strengthened Aged Care Quality Standards and relevant legislation.
- Deliver care that promotes independence, reablement, and wellness, supporting clients to remain living safely in their own homes.
- Provide personal care, domestic assistance, respite, and social support in line with assessed needs and preferences.
- Deliver care that is culturally safe, inclusive, and responsive to each client’s identity, values, and diversity.
- Uphold and support client rights in accordance with the Aged Care Charter of Rights and maintain compliance with the Aged Care Code of Conduct.
- Work in partnership with clients, families, and carers to support informed decision-making and consent.
- Deliver care within scope of practice, ensuring it is safe, evidence-based, and in line with organisational policies and legal requirements.
- Monitor, identify, and report changes in client condition, including risks relating to hydration, nutrition, mobility, cognition, pain, continence, and skin integrity.
- Recognise and respond to deterioration, escalating concerns in a timely manner according to procedures.
- Assist with medication management where delegated and within scope, in accordance with legislation and policy.
- Identify, manage, and report incidents, risks, and hazards in line with incident management systems and Serious Incident Response Scheme (SIRS) requirements.
- Follow infection prevention and control practices, including appropriate use of PPE.
- Adhere to Work Health and Safety (WHS) requirements, including lone worker safety procedures.
- Maintain accurate, objective, and timely documentation in accordance with legal, professional, and organisational requirements.
- Ensure confidentiality and privacy in line with the Privacy Act 1988.
- Participate in care planning, assessment, implementation, and evaluation within scope of practice.
- Provide clear and structured handovers to support continuity of care.
- Establish and maintain professional relationships with clients, families, carers, and the multidisciplinary team.
- Communicate effectively and respectfully with people from diverse backgrounds.
- Attend team meetings, supervision, and training as required.
- Collaborate with the multidisciplinary team to achieve safe and coordinated care outcomes.
- Participate in continuous quality improvement activities aligned with the Aged Care Quality Standards.
- Contribute to a culture of safety, open disclosure, and continuous improvement.
- Engage in ongoing professional development, training, and self-reflection.
- Support organisational requirements, including Direct Care Minutes, and provide feedback on service delivery.

- Deliver care that is person-centred, rights-based, and aligned with the Aged Care Quality Standards.
- Treat all clients, families, carers, visitors, and colleagues with dignity, respect, and compassion.
- Support clients to exercise choice, independence, and control over their care and daily lives.
- Actively involve clients and their representatives in care planning, goal setting, and decision-making.
- Obtain and respect informed consent for all care and services.
- Communicate clearly using language that is easy to understand and appropriate to individual needs.
- Recognise and respect cultural, spiritual, social, and individual diversity.
- Ensure care is inclusive and free from discrimination, abuse, or neglect.
- Support clients to understand and manage their health and wellbeing.
- Encourage social connection, emotional wellbeing, and community participation.
Key Selection Criteria
- Demonstrated understanding of and commitment to the Aged Care Quality Standards, Aged Care Charter of Rights, and Aged Care Code of Conduct.
- Knowledge of relevant legislation including the Aged Care Act 1997, Privacy Act 1988, WHS requirements, and SIRS obligations.
- Ability to work independently in a home environment with minimal supervision.
- Experience in providing personal care and domestic assistance to older people and/or people with disability.
- Demonstrated ability to apply wellness, reablement, and person-centred care approaches.
- Ability to recognise and respond to changes in client condition and escalate appropriately.
- Strong communication and interpersonal skills, with the ability to engage respectfully with diverse clients and teams.
- Understanding of professional boundaries, duty of care, and ethical practice.
- Basic computer skills to complete documentation and mandatory training.
- Commitment to continuous improvement, learning, and safe, high-quality care delivery.

YDHS is responsible for employing and contracting applicants who meet stringent community and public and public sector expectations. Applicants shall be required to provide and maintain:
- An original National Police Record Check completed within the last twelve (12) months prior to commencement.
- An employee Working with Children Check.
- A NDIS Worker Screening Check.
- Relevant professional registrations.
- Complete a Pre-existing injury declaration form.
- Undergo any other relevant checks, education or licencing as directed at own cost.
