Position Description

The Leisure and Lifestyle Coordinator – Planned Activity Group (PAG) is responsible for the coordination, planning, delivery and review of the Planned Activity Group and related leisure and lifestyle programs for aged, disabled and/or socially isolated members of the community. The role promotes physical, intellectual, emotional, cultural and social wellbeing, independence, choice, community participation and quality of life through personalised individual and group activities, programs and outings.

The coordinator works within a wellness and reablement, person-centred approach and ensures that PAG operates in accordance with relevant legislation, aged care and community care requirements, YDHS policies and procedures, quality standards, privacy and confidentiality requirements, and work health and safety obligations.

 

Key Responsibilities

 

  • Coordinate the day-to-day operation of the Planned Activity Group, including planning, organisation, delivery and review of activities, programs and outings.
  • Assess and respond to the interests, needs and preferences of clients, including their social history, cultural background, spiritual considerations, general health status and individual goals.
  • Develop, implement and evaluate individual and group leisure, lifestyle, social, recreational, diversional and intellectual activities that support wellbeing, independence and quality of life.
  • Provide clients with meaningful opportunities for choice, participation and self-expression, supporting them to maintain previous interests and develop new interests.
  • Support clients to maintain connections with their community and facilitate appropriate community participation and outings.
  • Coordinate and support volunteers and students involved in the PAG, including providing direction, orientation and practical support as appropriate.
  • Work collaboratively with relevant YDHS staff and services to support a coordinated, person-centred approach to client care.
  • Monitor the effectiveness and suitability of programs and activities and make recommendations for improvement based on client feedback, participation and identified needs.
  • Complete required administration, documentation, care/activity records, data entry and other reporting associated with the service.
  • Maintain client records in accordance with legislative, departmental and YDHS requirements.
  • Contribute to quality improvement activities, audits, surveys and service evaluation within relevant YDHS quality frameworks and Aged Care Quality Standards.
  • Identify, manage and escalate risks, hazards, incidents, changes in client needs and other concerns in accordance with YDHS procedures.
  • Promote and uphold client rights to privacy, confidentiality, autonomy, dignity, choice and safety at all times.
  • Ensure activities and services are delivered safely, inclusively and within the scope of the role.
  • Participate in performance review, mandatory education, professional development and training relevant to the position
  • Undertake other duties as directed that are within the scope and level of the position.

Key Selection Criteria

 

Mandatory:

  • Certificate III or IV in Leisure and Lifestyle, Aged Care, Diversional Therapy or a similar relevant qualification, or willingness to obtain appropriate training.
  • Demonstrated experience in developing, coordinating and delivering individual and group social, recreational, leisure, lifestyle, diversional and/or intellectual activities for older people and/or people with disability.
  • Experience working with clients who may be frail, aged, disabled or socially isolated, with an understanding of person-centered and wellness approaches.
  • Demonstrated ability to coordinate programs, organise activities and outings, prioritise competing requirements and work effectively with limited supervision.
  • Demonstrated ability to work collaboratively as part of a multidisciplinary/community care team, including taking direction and using initiative.
  • Strong verbal and written communication and interpersonal skills.
  • Competence with Microsoft Office, including Outlook, Word and Excel, and ability to complete required documentation and data entry.
  • Current Victorian Driver’s License.
  • Current First Aid Certificate.

Desirable:

  • Experience in a Planned Activity Group, Social Support Group or similar community-based program.
  • Experience coordinating or supporting volunteers and/or students.
  • Food Handler’s Certificate.
  • Experience working with people from diverse cultural and social backgrounds.

YDHS is responsible for employing and contracting applicants who meet stringent community and public and public sector expectations. Applicants shall be required to provide and maintain:

  • An original National Police Record Check completed within the last twelve (12) months prior to commencement.
  • An employee Working with Children Check.
  • A NDIS Worker Screening Check.
  • Relevant professional registrations.
  • Complete a Pre-existing injury declaration form.
  • Undergo any other relevant checks, education or licencing as directed at own cost.
