Better@Home is seeking interested clinician Enrolled or Registered Nurse to join their team on this fixed term contract. This position will report to Better@Home Manager.

The aim of the position is to provide support patients across their care continuum, working with patients & their care teams, identifying strategies or services to reduce the need to come to hospital.

The suitable candidate must;

•      Excellent communication skills an ability to develop rapport with people quickly
•      Be able to work independently but also collaboratively across care settings within a multi-disciplinary team
•      Good Time Management
•      Team Player
•      Energic & passionate to achieve better health outcomes
•      Motivate patients to work towards identified goals
