❤️ Care Partner – Support at Home

Central Gippsland Health (CGH) | Sale, Victoria

🌟 More than a job. A chance to change lives.

Help people live life on their terms. Build relationships that matter. Make independence possible.

What if your work could help someone stay in the home they love, remain connected to their community and continue doing the things that make life meaningful?

As a Care Partner with Central Gippsland Health (CGH), you can make that difference every day.

We're looking for a compassionate, proactive and solutions-focused person who loves working with people, building genuine relationships and finding ways to make things happen.

This is your opportunity to go beyond coordinating services. You'll get to know each client, understand what matters most to them and help turn their individual goals into practical, personalised support.

If you believe great care starts with listening, understanding and empowering people, we'd love you to be part of our team.

💚 Why you'll love this role

No two people are the same, and neither are their support needs. That's what makes this role so rewarding.

❤️ Make a real difference every day

Help older people and people living with disability maintain their independence, dignity, choice and connection to their community.

🤝 Build relationships that matter

Become a trusted partner to clients, their families and carers, building relationships based on understanding, respect and trust.

🏡 Help people live life their way

Support people to remain in the homes they love, stay connected to their communities and continue doing the things that matter most to them.

💡 Be a problem-solver and advocate

Think creatively, navigate challenges and find practical solutions that put each person's needs, goals and preferences first.

✨ Enjoy variety and autonomy

Bring together care coordination, service planning, advocacy, relationship management and budget oversight in a role where your professional judgement matters.

🌿 Be part of a supportive team

Collaborate with colleagues across health, aged care and community services to achieve the best possible outcomes for clients.

📚 Keep learning and growing

Build your skills and knowledge in the evolving Support at Home environment while contributing to high-quality, person-centred care.

🎁 Enjoy additional benefits

Access salary packaging, our Employee Assistance Program (EAP) and Social Club membership.

🏡 About the role: Putting people at the centre

As a Care Partner, you'll work alongside clients and their families to understand their circumstances, identify their goals and develop support arrangements that help them live as independently as possible.

You'll be the person who helps connect the dots — bringing together the right services, coordinating providers, navigating challenges and ensuring support continues to meet each client's changing needs.

Your responsibilities will include:

🤝 Building trusted relationships with clients, families and carers, taking the time to understand their individual circumstances and aspirations.

🎯 Developing personalised support plans that reflect each client's needs, preferences and goals.

🔗 Coordinating services and supports by working collaboratively with internal teams and external providers.

🧭 Guiding clients through Support at Home so they can understand their options, make informed choices and navigate the system with confidence.

💰 Managing individualised budgets by developing and monitoring budgets, explaining available supports and associated costs, and promoting transparency.

📋 Maintaining quality and compliance through accurate documentation, effective service coordination and adherence to relevant requirements.

🔄 Monitoring outcomes and adapting support as clients' needs, circumstances and goals evolve.

This is a role for someone who can balance compassion with sound judgement, attention to detail and practical decision-making.

You'll need to be comfortable navigating complexity, managing competing priorities and working collaboratively to achieve the best possible outcomes for each client.

Most importantly, you'll understand that person-centred care means starting with the person — not the paperwork.

🚀 Be part of the future of community care

The Australian Government's Support at Home program commenced on 1 November 2025, replacing Home Care Packages and introducing a new approach to delivering and funding support for older people living at home.

Care Partners play an important role in helping clients navigate this environment, understand their options and access support that reflects their individual needs and goals.

Joining CGH means contributing to a service that helps people remain independent, make informed choices and continue living life their way.

It's an opportunity to combine meaningful human connection with coordination, advocacy, planning and problem-solving — while helping shape positive experiences of community care.
