Central Gippsland Health Service (CGHS) is seeking a passionate and culturally knowledgeable Koori Hospital Liaison Officer to join our Social Health Team. About the Role

As the Koori Hospital Liaison Officer, you will play a vital role in supporting Aboriginal and Torres Strait Islander patients and their families throughout their hospital journey. You will help ensure care is culturally safe, respectful and responsive to the needs of patients, families and community.

Your responsibilities will include:

• Supporting Aboriginal and Torres Strait Islander patients and their families throughout their hospital journey.
• Advocating for culturally appropriate care and improved health outcomes.
• Assisting with care coordination, discharge planning and connections to ongoing support.
• Promoting cultural awareness, education and leadership across the organisation.
• Building and strengthening partnerships with local Aboriginal organisations, including Ramahyuck District Aboriginal Corporation.
• Participating in community events, including NAIDOC Week, and contributing to relevant advisory committees.

This is a largely autonomous role where you can take initiative, build trusted relationships and make a meaningful contribution, with support from the Social Health Team and opportunities to collaborate across a range of services.

You are a compassionate and community-minded person who understands the importance of culturally safe healthcare and is committed to improving outcomes for Aboriginal and Torres Strait Islander people.

• Strong interpersonal and communication skills.
• The ability to work independently while contributing effectively to a multidisciplinary team.
• Confidence engaging with community organisations and supporting collaborative approaches to care.

Mandatory requirements:

• Certificate IV in Aboriginal and/or Torres Strait Islander Primary Health Care, or equivalent.
• Current driver's licence.
• Demonstrated cultural knowledge and community engagement skills.
• Strong interpersonal and communication skills.

Central Gippsland Health (CGH) is a sub-regional health service and the major provider of health and aged care services in the Wellington Shire.

Located in Sale, approximately 215 kilometres east of Melbourne, CGH offers access to the beautiful Gippsland Lakes, Ninety Mile Beach and the Victorian Alps.

We deliver integrated healthcare across three campuses in Sale, Maffra and Heyfield, providing a broad range of acute, sub-acute, aged care and community services. Our acute services include a 24-hour emergency department, critical care, neonatal critical care, operating theatres, day procedures, oncology and dialysis, alongside general medical care and rehabilitation.

Our services extend beyond the hospital, with four residential aged care homes in Sale, Maffra and Heyfield, as well as community-based programs that help people remain healthy, active and independent in their own homes. These include Support at Home, Commonwealth Home Support Programs and other tailored in-home services.

Our multidisciplinary allied health team provides physiotherapy, dietetics, occupational therapy, speech pathology and social work, complemented by accessible public dental services.

CGH is also a teaching hospital affiliated with Monash University's School of Rural Health, supporting student placements and clinical learning opportunities.
