CGH provides acute care as part of an integrated service delivery model across Wellington Shire and beyond. The service provides beds across three campuses (Sale, Maffra and Heyfield).
Acute services include a full-time emergency department, critical care unit, neonatal critical care unit, three operating theatres, day procedure unit, oncology and dialysis services in addition to general medical and services and sub-acute services including rehabilitation.
CGH also provides aged care and community services in the Wellington Shire and beyond.
We operate four residential aged care homes located in Sale, Maffra, and Heyfield delivering high-quality, person-centred care in supportive and homelike environments.
CGH provides a wide range of community-based services that support people to remain healthy, active, and independent in their own homes. This includes the provision of Support at Home, Commonwealth Home Support Programs, and other in-home services tailored to individual needs.
Our integrated model of care is supported by a multidisciplinary allied health team offering physiotherapy, dietetics, occupational therapy, speech pathology, and social work. We also provide accessible public dental services to enhance community wellbeing.
CGH is also a leading teaching hospital, affiliated with Monash University's School of Rural Health and provides placements and clinical experience for students.

About Department
The Community Palliative Care Service is based in Sale and delivers care in a variety of settings across the Wellington Shire. Care is predominately home based however provides some support to the hospital and Aged Care Facilities.
About the Role
You will enable high quality, client focused Palliative Care Services through effective client care coordination and professional leadership; support the development of the clinical nursing staff within the Palliative Care Service; and provide leadership in achieving and upholding and fulfilling the organisations vision, mission and values.
Duties
· Work as part of a multidisciplinary team that supports palliative care service delivery; predominately in the home.
· Provide leadership and support for palliative care staff
· Provide leadership and advocacy for palliative care clients across the care continuum
· Balance competing priorities and work demands and to identify and respond appropriately to critical and urgent clinical needs.
· Support the management team in appropriately prioritising patient needs and to operate efficiently and effectively with a view to meeting community needs and providing a financially sustainable service.
Qualifications, Skills and Experience
· Degree Qualification in Palliative Care
· Registered Nurse holding a current Australian Registration Certificate with AHPRA
· Proven experience working as a palliative care nurse in a senior role.
· Current Victorian Driver’s License
